Students planning on taking the PSAT, SAT, ACT or Advanced Placement tests who have a documented disability may be eligible for accommodations on these tests. However, a form completed by both the school and the parent must be completed in order for eligibility to be determined by the Collegeboard and/or ACT organizations. If you would like to apply for accommodations on any of these tests, please have your student stop by Office 153 to pick up a Student Eligibility Form.
Note: A separate form needs to be completed for the Collegeboard and ACT if your student plans to take both the SAT and the ACT.
